Telehealth Services

By engaging in telehealth services, you consent to the collection and use of your information in this manner, acknowledging the inherent risks of electronic communication.

Workplace Physiotherapy

Information sharing with employers occurs only with your consent or where legally required, and is limited to necessary details such as work capacity and functional restrictions.

Website Data

IP addresses, browser information, and usage data are automatically collected. Cookies are used to optimise your experience and can be managed through your browser preferences.

Information Sharing

Where appropriate or required, your data may be shared with GPs, other healthcare providers, ACC, employers, insurers, professional advisors, IT providers, and government agencies when legally required or authorised.

Security Measures

Access to your information is restricted to authorised personnel. Our systems are protected against unauthorised access with regular security reviews. Information is securely destroyed when no longer needed.

Retention Period

Health information is retained in accordance with New Zealand legislation, which generally requires records to be kept for at least 10 years from the last date of service.

Access and Correction Rights

You may request access to your information by contacting candice@wayforwardphysio.co.nz. We aim to respond within 20 working days. Factually incorrect information will be updated; where a clinical opinion is disputed, a correction statement will be added to the record.

Complaints

If you have a concern about how your information has been handled, please contact us first so we can address it. Unresolved concerns may be escalated to the Office of the Privacy Commissioner (New Zealand).
